• 62 year old male is a full time part time employee on disability with Medicare coverage.

• Actually looking for quotes for self employment health insurance for myself, my wife & 2 children.

• Aetna just cancelled its contract with Morton Plant Mease hospitals. These hospitals are a preference for my employees. Also, they want to be able to get medical care when they are out of town.

• Also interested in hdhps, hsas, and fsas.

• An insurance plan with an HSA savings account might be a good option if a high deductible plan is used.

• Company paid the health care insurance, no deduction to employees. We need a special price.

• What we are looking for is: A fairly low deductible $1000 -- $1500, a PPO, a office visit co-pay of $20-$40 an 80/20 plan is fine, and a prescription coverage like $10/$15/$25 etc. Thank you.
