62 year old male is a full time part time employee on disability with Medicare coverage.

Aetna just cancelled its contract with Morton Plant Mease hospitals. These hospitals are a preference for my employees. Also, they want to be able to get medical care when they are out of town.

We have been in business as a sole proprietorship for 9 years and are converting to an LLC in Jan 2009. I am assuming we have to be an LLC to offer the health insurance -- true? Or can we do it as a sole proprietorship? And if we started now, as a sole proprietorship, could we keep the same insurance policy when we convert to an LLC (and change our name, but same owner(s))? If we cannot keep the same policy, then we would wait until Jan when we become an LLC.
We need a strong prescription plan that is flexible and carries many types of drugs including what are considered lifestyle drugs. Lower in co-pay for prescriptions both generic and non-generic drugs.

We would like various options on our health plan. High and low deductibles.
What we are looking for is: A fairly low deductible $1000 -- $1500, a PPO, a office visit co-pay of $20-$40 an 80/20 plan is fine, and a prescription coverage like $10/$15/$25 etc. Thank you.
